If you already have assigned funding — whether you’ve transitioned from a Home Care Package or started directly under Support at Home — your budget is designed to give you flexibility and choice.
You can use your funding for the services that best support your lifestyle and wellbeing. Your Aunty Grace Care Partner can help you review your care plan and make sure your funding is being used in the most effective way.
For example, if you currently receive garden maintenance services, you might find that in Spring your garden grows more quickly and needs extra care. Your Care Partner can help you rework your budget to include additional visits, or explore other supports such as domestic assistance to help around the home.
It’s all about ensuring your funding reflects what you need most — keeping you safe, comfortable, and independent at home.
If you have a prescriptive support plan from the government, your services may be limited to what’s listed in your plan. In that case, it may be time to think about reviewing your support plan or reassessment options.
If your current services aren’t meeting your needs, or your situation has changed, you can request a support plan review or reassessment.
This process helps ensure your Support at Home classification and funding level accurately match your current needs. If you’re with Aunty Grace, your Care Partner can assist you in requesting this review and guide you through each step.
An assessor will review your support plan and assess your needs. Based on this assessment, your funding or service classification may change to reflect your updated situation. If additional funding or services are approved, your Aunty Grace Care Partner will help you update your care plan and ensure your spending remains balanced — maximising your support while staying within your budget.

Beyond your regular Support at Home budget, there are additional funding options available to help you live safely and comfortably at home. These funding pathways are separate and need to be accessed individually — but don’t worry, your Care Partner will guide you through the process.
Assistive Technology and Home Modifications (AT-HM)
The AT-HM Scheme can help you access funding for assistive technology and home modifications — things like mobility aids, safety equipment, or home adjustments to make everyday life easier. Funding amounts vary depending on your needs, and you may be asked to contribute toward some costs.

Restorative Care and End-of-Life Care
You may also be eligible for additional funding under the Restorative Care Pathway, which provides short-term support (usually around six weeks) to help you regain independence after an illness or setback.
For those who need extra comfort and care during later stages of life, the End-of-Life Pathway offers additional funding and services for around twelve weeks, helping you and your loved ones focus on what matters most.
