The Support at Home program has some key eligibility criteria. We've put together the following information to help you better understand if you, or a loved one, are eligible. And what the application process looks like from here. We’re here to make the process simple for you.
Eligibility for the Support at Home program is for Australians who are:
And need support with everyday tasks associated with living at home due to age, frailty, or ongoing health requirements.
Note: People over the age of 50 who are at risk of, or are currently experiencing homelessness, may also be eligible under certain circumstances. In this context, "living at home" broadly refers to living in the community (i.e. not in a residential aged care facility), and includes various temporary or unstable living situations.
Your eligibility for the program is assessed by My Aged Care. It involves an application and usually an in-person assessment. The assessment is done through the Aged Care Assessment Team (ACAT).
It uses the new Integrated Assessment Tool (IAT). The tool was introduced in July 2024 to replace the previous National Aged Care Screening and Assessment Form. It streamlines eligibility assessments by ensuring older Australians only need to provide certain types of information once to reduce confusion and delays in support.
The Support at Home program is set to start rolling out across Australia from November 1st, 2025.
If you're already on a Home Care Package, you'll automatically transition across. There’s no need to reapply - you'll continue to receive the same level of government-funded support you enjoy now, but with more options and flexibility.
New participants who are assigned their government funding from November 1st 2025 will go straight into Support at Home.
The Aunty Grace family is already preparing for the transition to ensure there are no disruptions to your care. Your dedicated Care Manager will be there to guide you through the process. We'll make sure you receive access to all the services you’ll need under the new system.
If you’re already on a Home Care Package, you don’t need to do anything.
If you are a new applicant, the current Home Care Package application process still applies up until October 31st 2025. After that date, the new application process comes into effect.
Either way, contact My Aged Care on 1800 200 422 or through their website to start your eligibility assessment process.
Once approved, you can nominate Aunty Grace as your provider, and we’ll take it from there. We will arrange everything for you, including selecting the right care services and navigating the funding process.
Your Care Manager will also explain everything clearly, and make sure your arrangements are handled with the utmost attention to detail.
