This program typically runs for 6 to 12 weeks and provides intensive, coordinated support from allied health and nursing professionals. It’s especially helpful if you’re transitioning home after a hospital stay, managing a temporary illness or injury, or need focused support to improve your wellbeing.

 

Restorative care focuses on helping you achieve personal goals — whether that’s improving mobility, increasing strength, or regaining independence in daily tasks — with the aim of delaying or reducing the need for higher levels of care.

4. End-of-Life Care

 

The End-of-Life Pathway provides additional funding and support for older people who have been diagnosed with three months or less to live and wish to remain at home.

 

This funding, typically available for up to 12 weeks, helps cover personal care, nursing, and other in-home services to ensure comfort, dignity, and peace during this time.
