One of our Expert Care Partners will come and meet you to tailor your Home Care Plan - when you join Aunty Grace, your local Care Partner will visit you in your home to understand your needs and comprehensively assess your home environment. The new Support at Home program Commenced on November 1st 2025, if you’re currently on a Home Care Package Level, Short-Term Restorative Care or still applying, you are now under this program.
