After your assessment, you’ll be informed of the result and the level of support that has been approved. Once you know how things have gone, simply let us know — staying in touch means we can help guide you through what comes next.

Ready to begin when funding is allocated
Once your Support at Home funding is allocated, we’ll be ready to get services started as soon as possible, with care designed around you at home.

In-home visit & support plan
If you’d like to move forward, we can organise an in-home meeting to create a tailored support plan that works around your schedule, goals, and level of independence.. ready for when the funding is allocated! Once care begins, our team stays closely involved — providing ongoing check-ins, adjustments as needs change, and genuine support every step of the way
