One of our expert care managers will come and meet you to tailor your home care plan - when you join Aunty Grace, your local care manager will visit you in your home to understand your needs and comprehensively assess your home environment. When your circumstances change, we'll come out to see you again. Your care manager will regularly check in to make sure we never miss an opportunity to better support you.

Working with your care manager you will also have comprehensive care team - each client receives a dedicated team, including a care manager for personalized service planning, a service coordinator for service management, and accessible concierge support for all needs.
