From clinical care to everyday living support, from assistive technology to home modifications, we make sure your services are tailored to you and that your funding is working hard for you.

Support at Home services are grouped into five key areas — all designed to help you live life your way.
These services help you manage your health at home, with no out-of-pocket costs. Delivered by qualified professionals, they’re tailored to your health conditions, goals, and lifestyle.

Some services may require a contribution payment. We’ll guide you through what this means and help arrange the right support.
The AT-HM Scheme is here to help you live safely and comfortably at home. If you’re eligible, you’ll receive funding for assistive technology, home modifications, or both—giving you the support you need to stay independent.
This funding is available for 12 months, and you may be asked to contribute towards some costs. If you haven’t accessed this funding before or aren’t yet approved, we’ll help you get assessed.

With Aunty Grace, getting support doesn’t mean giving up you or your loved one's independence. Your Care Partner will regularly talk with you about practical ways to stay supported and independent. They can connect you with services to support you and your loved one, all in line with your Support Plan.
