Through the Support at Home program, we offer a wide range of services that keep you safe, healthy and happy at home. From clinical care to everyday living support, from assistive technology to home modifications, we make sure your services are tailored to you and that your funding is working hard for you.

What services can you access?

Support at Home services are grouped into five key areas — all designed to help you live life your way.

Clinical support

These services help you manage your health at home, with no out-of-pocket costs. Delivered by qualified professionals, they’re tailored to your health conditions, goals, and lifestyle.

Nursing In-home care including wound management, medication support, and chronic condition monitoring. Helps you stay on top of your health without hospital visits.
Podiatry Foot care including nail trimming, callus removal, and diabetic foot checks. Supports mobility and prevents complications.
Occupational Therapy Assessments and strategies to help with daily tasks and home safety. Promotes independence and reduces risk of injury.
Physiotherapy Exercises and treatments to improve strength, balance, and movement. Helps maintain mobility and prevent falls.
Speech Pathology Support for communication, swallowing, and cognitive challenges. Improves quality of life and safety during meals.
Rehabilitation Services Recovery support after surgery, illness, or injury. Speeds up recovery and helps regain independence.
Nutrition & Dietary Needs Meal planning and advice tailored to health conditions. Ensures proper nutrition and supports overall wellbeing.
Specialised Supports Services for hearing, vision, cognition, and other medical needs. Addresses specific challenges to improve daily living.

Some independence services may include a small contribution under the Support at Home program but it’s a great way to make the most of your funding. For many people, this small cost unlocks big benefits in staying active, mobile, and connected.

Social Support & Community Engagement Visits, outings, and group activities. Keeps you socially connected and emotionally well.
Respite Temporary care to give family carers a break. Supports carer wellbeing and prevents burnout.
Transport Safe rides to appointments, shops, or social events. Encourages independence and community participation.

Everyday living

These services help with daily tasks so life runs smoothly. Whether it’s cooking, cleaning, or gardening, we’re here to make home life easier.

Domestic Assistance Help with cleaning, laundry, and household chores. Keeps your home safe, clean, and comfortable.
Home Maintenance & Repairs Minor fixes and safety checks around the house. Prevents hazards and keeps your home in good shape.
Gardening Lawn care, pruning, and outdoor upkeep. Supports mobility and enjoyment of outdoor spaces.
Meal Solutions Help with shopping, meal prep, or ready-made meals.  

Some services may require a contribution payment. We’ll guide you through what this means and help arrange the right support.

Assistive Technology and Home Modifications

The AT-HM Scheme is here to help you live safely and comfortably at home. If you’re eligible, you’ll receive funding for assistive technology, home modifications, or both—giving you the support you need to stay independent.

This funding is available for 12 months, and you may be asked to contribute towards some costs. If you haven’t accessed this funding before or aren’t yet approved, we’ll help you get assessed.

Gentle, respectful care tailored to memory and cognitive needs. Supports safety, dignity, and comfort for those living with dementia. 

At Aunty Grace, we understand the unique challenges of living with dementia. Our Care Team is trained in dementia care and knows how to provide thoughtful, effective support that respects each person’s needs and preferences. We make sure your workers understand any concerns you or your loved ones may have, and we connect you with the right services to help you live well with dementia at home.

Meal planning and dietary advice based on your health needs. Helps maintain energy, manage conditions, and enjoy mealtimes.

Aunty Grace clients get access to locally produced meals made with love by a selection of approved providers. We make it easy for you to choose the meals that you like to eat, delivered directly to your door.
We know how important it is to be able to access high quality, nutritious meals.

Social connection

Visits, outings, and programs to keep you engaged. Reduces loneliness and improves emotional wellbeing. 

Social isolation is associated with poorer health as we get older, so at Aunty Grace we make it a priority to ensure you’re part of a community that supports you. This includes: 

  • Companionship, and group social & exercise services, where permitted under your Support Plan
  • Regular check-ins and conversations with our Care Team
  • Involving your family and loved ones in your care
  • Finding workers you can build a great working relationship with, and ensuring that services are delivered in a friendly and engaging way
  • Attending one of our famous Aunty Grace get-togethers!

With Aunty Grace, getting support doesn’t mean giving up you or your loved one's independence. Your Care Partner will regularly talk with you about practical ways to stay supported and independent. They can connect you with services to support you and your loved one, all in line with your Support Plan.
